By signing below, I understand that it is my responsibility to fully test and validate the equipment, I am stating that I have done so, and I am approving this equipment "as is", with the exception of requested changes listed in the notes column above. I have the appropriate authority to accept this equipment on behalf of my company. I understand that my signature constitutes as approval to move into the warranty phase of this project, which includes final payment for the project.
I understand that APT Manufacturing Solutions is available for start-up support as this line is integrated into our production process. If start-up support hours were purchased as part of the initial project, those hours will be reduced going forward. Additional start-up support hours are available to purchase as needed.
I understand that the warranty period now begins for this equipment, and that I will need to communicate any issues to APT as soon as they are discovered in order to be resolved expeditiously.
